This field-based role provides direct medical care to unhoused individuals in outdoor spaces, including streets, parks, near public transportation, and encampments across North LA County.

As an RN on the Street Medicine team, you will never work alone—you will always be part of a multidisciplinary team that includes substance use specialists, mental health clinicians, and peer specialists with lived experience. About The Role
As an RN on the Street Medicine team, you will never work alone—you will always be part of a multidisciplinary team that includes substance use specialists, mental health clinicians, and peer specialists with lived experience. The goal is to provide comprehensive, compassionate care in the community to address chronic conditions, mental health concerns, and substance use issues.
Key Responsibilities
- Conduct outreach visits in encampments, parks, and other locations using a personal vehicle (locations include Altadena, Glendale, West Covina, El Monte, Pomona, and areas along the foothills to the San Bernardino County line).
- Perform field phlebotomy, wound care, and minor procedures in outdoor environments.
- Provide chronic disease management, substance use care, and mental health support.
- Travel on foot to access certain encampments (including hiking, as needed).
- Carry and manage medical supplies, as there are no designated treatment rooms or facilities.
- Verify Medi-Cal eligibility, assist patients in switching health plans, and help them enroll in clinical services.
- Build trusting relationships with patients to engage them in ongoing care.
- Work closely with case managers and primary care providers to ensure coordinated treatment plans.
- Educate and empower patients to make informed decisions about their health.
- Document visits and maintain compliance with healthcare regulations.
- Current, active California RN license (unencumbered).
- Minimum 2 years of experience working directly with patients in a clinical, public health, or field-based setting.
- Field phlebotomy experience strongly preferred.
- Experience working with individuals experiencing homelessness highly desirable.
- Background in substance use treatment, mental health care, or community outreach preferred.
- Ability to walk long distances and hike to certain encampments while carrying medical supplies.
- Strong interpersonal skills and a commitment to culturally sensitive care.
- Comfort with insurance navigation (Medi-Cal eligibility verification, health plan switching).
- Reliable personal vehicle, valid CA driver’s license, and state liability insurance.
- Monday – Friday, 8:00 AM – 5:00 PM .
- Supplies are shipped to your home for field use.
- Must provide proof of Tuberculosis screening upon hire (or within the last year).
- Ability to remain calm and resourceful in emergency situations.
- Must adhere to HIPAA regulations and maintain patient confidentiality.
